In this folder, you will find a first-draft of an Informed Consent that was developed for a local agency years ago. The agency was applying for a federal grant, and needed to conduct a needs assessment of youth in Gulfton-area (predominately Spanish-speaking community) middle and high schools in Houston, Texas. This was the first-draft of the Informed Consent, which obviously, needed a lot of revisions.
