Real-World Case 4.1

The hospital clinical documentation improvement (CDI) specialist reports to you, the HIM manager of Anywhere hospital, that the hospital has been receiving reimbursement penalties. The physician documentation is not appropriately identifying specific conditions that CMS has identified as hospital-acquired conditions (HACs). In most cases, these conditions are present in patients before they are admitted. However, physicians are documenting these conditions later in the patients’ stays, making it appear that the patients have ­acquired these conditions from the hospital. The hospital is being financially penalized because these conditions are considered to be preventable if the hospital follows national treatment standards and guidelines.

 

Anywhere hospital has a robust EHR. Many ­documentation improvement initiatives have been leveraged by the technological capabilities of the EHR. As the HIM manager, you wonder if something could be done from a technology standpoint that could assist physicians with identifying, upon admission, those conditions that are causing the reimbursement issue, and appropriately documenting the conditions. Physicians appropriately capturing and documenting these conditions would demonstrate that the hospital is following the national treatment standards and guidelines and the patients are not acquiring these conditions in the hospital.

A visual analysis is a close inspection of the artist’s use of art elements and design principles. A visual analysis moves beyond pure description. It is intended to enhance your observational and analytical skills, asking you to think critically about the choices that an artist makes. You will explain how an artist uses line, color, texture, space, emphasis, balance, proportion, and so forth, to create a unique composition. You will want to provide a detailed description of the formal (meaning “related to the form”) characteristics of the artistic composition. Remember you’re not just describing what you see, you’re describing what these elements do in the work.

Some things to consider in your analysis:

• Line: Are there any prominent lines in the work? Do you see psychological or implied lines? If so, do they serve any compositional purpose? Is line used to create a contour of an object or exist in its own right? Are the lines mostly horizontal, vertical, or diagonal? What do they suggest?

• Shape: What shapes do you see? Are they geometric? organic? biomorphic? Are there well-defined figure and ground shapes? Is shape a prominent aspect of the work?

• Mass: Does this work have real or implied mass? Where do you see this in the work? What indicates mass?

• Light: How is light used in the work? is there real or implied light? What are the values like? Do the values span white to black? Is the image relatively light or dark?

• Color: What types of colors are used? Are they warm or cool, bright or dull? Are they natural or arbitrary? Are the colors organized to lead your eye in a particular direction? Is color used to create space? What is the overall effect of color?

• Space: Is implied space created? If so, which pictorial devices are used to render the illusion of three dimensions on the two-dimensional plane. Does the artist use linear perspective, atmospheric perspective, vertical placement, overlapping, etc.? Are line and color used to create the illusion of mass or volume?

• Texture: Is there actual texture (impasto) or implied texture? Can you see the artist’s brushstrokes? Or, is the artist’s hand totally concealed? What is the overall effect of texture?

• Time and motion: Is there an implied time of day/year/era in history? Do you see implied movement or real movement? What is the effect of these elements to how we see the work?

• Design: How are the formal elements organized? What is the focal point of this composition? Describe how the artist creates emphasis in this work and

 

 

why your eye moves to this area of interest. Is the work symmetrical or asymmetrical?

Do not try to explain the subject, interpret the meaning, or judge the quality of the work. Instead, think about what you see. As you write your analysis, remember that the goal is to sharpen your visual literacy skills. You want to focus on the formal elements and design principles the artist used to create the composition (not on the meaning of the work). The Infographic: Choose one of the 4 images provided in the Infographic Images folder, under the Visual Formal Analysis folder. Design an infographic that highlights 4-6 main visual characteristics of your chosen artwork. Focus on the ones that are most prominent in the work. Your infographic should include the image with artist’s name and information. All information should be concise, grammatically correct, and easy to read. The information should be organized well. Think of this assignment as designing an informational poster for a conference. Your poster should be well designed and informative. Democratic Presidential Nominee: Last year, when Joe Biden cemented his status as the presumptive Democratic Presidential nominee, his work on the 1994 Violent Crime Control and Law Enforcement Act became the subject of deserved—if somewhat belated—criticism. While the intervening years have revealed it to be a highly problematic piece of legislation, those of us who were alive at the time remember the early 1990s as an era during which anxiety over violent crime seemed to be at an all-time high. Even worse, many criminologists at the time had predicted that things would only get worse.

 

The long-promised crime wave, however, never materialized. On the contrary, data would later reveal that violent crime began to decline in 1990 and plummeted throughout the decade. Here’s the thing, though: no one knows exactly why. Just about everyone in politics, law enforcement, and/or corrections has tried to take credit at one point or another, but none of their explanations match up with the data. Not all of the data, anyway.

 

Your mission, should you choose to accept it (as if you had a choice), is to use what you learned from the chapter on causality and experimental design to evaluate two possible explanations.

 

The first possible explanation comes from Kevin Drum, a writer at the magazine Mother Jones. Although Drum is by no means a social scientist, he does a decent job of articulating the work of a handful of legitimate criminologists.

 

The second possible explanation comes from Stephen D. Leavitt, author of Freakonomics, among other things. For the record, I have a love/hate relationship with Leavitt’s work. His research ranges from laughable to offensive, but he does such a good job of explaining his methods that I can’t help but refer to his work in at least two of my classes. Also, you should note that this piece, which was first published in 1997, has not aged well. Just to give you an idea of how much the world has changed in the interim, Rudy Guiliani was still considered a respectable politician when it initially hit shelves. Anyhow, I left some sarcastic notes in the margins, which you may or may not find helpful.

Your Assignment

 

After reading Drum’s article and the chapter from Leavitt’s book, you will write three blurbs, each of which should be approximately 250-500 words (yielding a total of about 750-1500 words). In the first blurb, you will use the criteria outlined in the textbook chapter to evaluate the extent to which Drum succeeds in establishing a causal relationship between lead and crime rates (6 points). In the second blurb, you will use those same criteria to evaluate the extent to which Leavitt succeeds in establishing a causal relationship between abortion in crime rates (6 points). Finally, in the third blurb, you will offer your opinion as to whether Drum or Leavitt makes the better case (8 points). The content of this third blurb should be based on the criteria necessary for establishing causality between two variables, not how you feel about one or both of the writers (or, for example, leaded gasoline).

Since social media can be different depending on the type of organization/business, your assignments will require you to do various exercises related to these four primary types of organizations:

· B2C (Business-to-consumer) – companies that sell to individual purchasers who buy and consume products/services for personal use.

· B2B (Business-to-Business) – companies that sell to other businesses or entities that use the products/services as part of daily business operations, or to create a final product they sell to consumers.

· Non-profit organization not operated by a government entity – organizations that generate funds, but use what they earn for some type of philanthropic, humanitarian, social or charitable purpose.

· Government agency or group – an entity that provides oversight and administration of specific functions as outlined by its government and is operated by its government. This can be at any level such as in the United States, it could be at a city, state or federal level.

 

The assignment for this week is to choose one business or organization for each of the four categories listed above. As you go through this course, certain assignments will ask you to apply what you learned to each of your chosen businesses. Because you will be working in context of your four organizations, I suggest you choose ones that you are interested in, since these organizations will be the base of several of your future assignments. A few things to note when making your selections:

· Once you choose these for businesses/organizations, you CANNOT change them.

A high-volume warehouse has over 100 order pickers. The company requires supervisors to track staff productivity to ensure that employees “make rate” or pack a minimum number of boxes per hour.  The system in use automatically tracks each staff member’s productivity by recording the time each box is finished packing when the staff member scans the barcode on the box.  The system automatically generates warnings that are routed to the employee when productivity falls below a set threshold as well as termination suggestions.  The bottom 5% of employees are automatically placed on a training plan.  Supervisors are responsible for ensuring that the warnings and terminations are accurate and appropriate action is taken such as reprimands, additional training, or termination activities.  The company has highlighted its high productivity and efficiency as a key differentiator in company success. The company has communicated its strict standards more widely. “Associates must be detailed and efficient in processing each order.”  To ensure that efficiency continues, the company has developed “a proprietary productivity metric.” The company says those goals are set objectively, and that they’re based on metrics like customer demand and location.
